The challenge is completed when you click x amount of times in the zones it tells you. You can reposition the girls by holding your mouse button down and moving them (same as you would move a file in the windows operating system), making her slightly smaller using the + - bar shown on your image (on the right side of screen) can help you see all of her and make it slightly easier to click the right area, try to remember the sequence of arears you have to click as these do not change. Once you have the 3rd perk (position 9) there is no point in adding more affection to her as it simply unlocks an extra image you would be better adding the affection to another girl to get closer to unlocking another perk.
