What is the item in the picture, how can I use it?

You have won what is called an avatar. 

To use, left click in the upper left corner of your screen in an orange box that has an image. 

Next, in the lower right you will see a blue circle with what looks like a pencil, click on that.

On the left hand side of your screen images will appear.  Click on the tab marked 'Avatar'.

These are the images you can choose from as your Avatar.  Some you can buy with 300 kobans.  The ones you have won you can use for free.

Use the right and left white arrows next to this box to scroll through the selections and choose which you like.

To exit, click the blue circle with the arrow within.

Thanks, I hadn't realized we could scroll through the list of avatars. I would say it's a design flaw because it's not obvious, but I guess they want to advertise the avatars, so on their end it's a feature.
